  • Depth Measurement – Measuring depth involves using a sterile probe, such as a cotton swab, to gently explore the deepest part of the wound. The probe is carefully marked at the level of the skin margin, and the measurement is taken from the tip to the marked level.

When recording depth, be sure to identify any wound undermining and tunneling.

Undermining occurs when the tissue under the wound edges becomes eroded, leading to a space or pocket beneath the skin surface. Tunneling refers to a channel or pathway that can develop within the wound and extend into deeper tissues. Check for undermining and tunneling at each “hour of the clock”, looking for depth and direction.
